Cagrilintide

Long-Acting Amylin Receptor Agonist

Low Dose
0.25 mg/wk (titration start)
Medium Dose
2.4 mg/wk
Route
SubQ weekly
Cycle Length
Ongoing

Mechanism of Action

Long-acting acylated amylin analog. Amylin, normally co-secreted with insulin, signals satiety via the area postrema/brainstem; cagrilintide's fatty-acid-acylation and albumin binding (the same technology as semaglutide) extend this satiety signal, complementing GLP-1's separate appetite pathway.

Key Benefits

  • Greater weight loss combined with semaglutide (CagriSema) than semaglutide alone (~20.4% vs ~3.0% placebo at 68 weeks, REDEFINE 1)
  • Distinct amylin-pathway mechanism complements GLP-1 monotherapy
  • Comparable overall safety profile to semaglutide monotherapy
  • Improved waist circumference and blood pressure in combination therapy

Dosing Protocol

Low / Starting Dose
0.25 mg/wk (titration start)
Medium / Maintenance Dose
2.4 mg/wk
Administration Route
SubQ weekly
Half-Life
~7 days
Frequency
Once weekly
Cycle Length
Ongoing
Protocol Notes

The overwhelming majority of efficacy/safety data is for the CagriSema combination with semaglutide, not cagrilintide monotherapy; titrate on a schedule similar to semaglutide.

Ideal Candidates

Adults with obesity or overweight with comorbidity, particularly those who plateaued on GLP-1 monotherapy alone.

Side Effects & Safety

GI adverse events (nausea, vomiting, diarrhea, constipation) more frequent than placebo, and more frequent than semaglutide alone in combination; injection site reactions.

Regulatory Status

Gray Area

Investigational — not yet FDA-approved as monotherapy or as the CagriSema combination. Novo Nordisk's REDEFINE program is under regulatory review as of 2026. No 503A listing.
